Screening Letters for Your Practice

The Hawaii IPA developed a set of reminder letters for members. The letters, in Wordª format, may be personalized to fit your needs and your office letterhead. The screening letters cover the following topics:

• Hemoglobin A1C screening
• Mammography
• Retinopathy screening
• Cholesterol
• Chlamydia question letter

Each letter informs your patient of the need for the suggested screening along with some statistics. The letter encourages their response and reassures them of the ease of the process and of complete patient confidentiality.
